The ESR test relies on a basic biological phenomenon: when inflammation occurs, the liver releases acute-phase proteins (like fibrinogen) into the bloodstream. These proteins reduce the natural repulsive charge of red blood cells, causing them to clump together into coin-like stacks called rouleaux. Because they are heavier, these clumps settle to the bottom of the tube more rapidly.
